Title: Coordinator for Care Management & Support Services
Salary: $62,000 - $65,000 annually
Status: Full-Time, Non-Exempt, 40 hours weekly
Supervisor: Chief Program Officer
Schedule: Flexible schedule to meet client needs
Function:
The Coordinator for Support Services will develop and coordinate programming that plays a vital role in supporting clients and staff within Family Services’ (FSI’s) behavioral health program, ensuring that Support Services are integrated seamlessly into our system of care. This position fosters partnerships with internal and external stakeholders - including Behavioral Health and Operations leadership, Health Home Care Management teams, SCN navigation professionals, Peer agencies, and other collaborators - to enhance the quality and accessibility of services for those we serve. The Coordinator will supervise program staff and ensure Support Services are high quality and meet all agency and funder goals. The Coordinator will also help prepare FSI for the future, helping the agency to integrate additional supportive services such as community health workers or internal peer support services into our system of care.
Essential Requirements
Bachelor’s degree in social work, psychology, public health, or a related field (Master’s preferred).
Experience coordinating or managing support services such as Health Home Care Management within behavioral health or healthcare settings.
Reliable transportation needed for occasional travel to partner sites or community locations.
Ability to remain in a stationary position (sitting or standing) for extended periods of time.
Ability to operate a computer and standard office equipment for prolonged periods.
Ability to move throughout the office and between workspaces as needed.
Ability to occasionally lift and/or move items up to 5 pounds (e.g., files, supplies, training materials).
Skills & Abilities:
Strong organizational, communication, and interpersonal skills.
Ability to build effective partnerships with diverse stakeholders including administrative staff, peer agencies, and community partners.
Understanding of and experience utilizing principles of change management for effective change and implementation of new program.

Position Responsibilities:
Coordinates and oversees the delivery of Health Home Care Management, SCN navigation, and peer support services for clients and staff.
Acts as a liaison between our behavioral health program, external peer agencies, and internal teams to facilitate collaborative care.
Engages stakeholders to develop strategies for integrating support services into the system of care, optimizing client outcomes and staff support.
Assists in the planning and implementation of new service offerings, such as community health workers and internal peer services, as the program evolves.
Monitors service quality and adherence to program standards and work with staff to ensure compliance with requirements and the highest quality care.
Provides training and guidance to staff on all service requirements, protocols and best practices.
Provides regular supervision, leadership and support for Support Services staff.
Collects and analyzes data, documentation and performance of program and staff, and implement quality improvement principles to achieve goals.
Participates in meetings and trainings relevant to program needs.
Other duties as assigned.
Qualifications:
Knowledge of Health Home Care Management, SCN navigation, and peer support models.
Proficiency in data collection, analysis, and quality improvement, with attention to detail.
Ability to meet agency and regulatory background check requirements.
